You may make your initial appointment directly through our hospital with a referral from your veterinarian who will provide us with your pet’s medical history. We will schedule an appointment at a convenient time when you and your family can comfortably discuss further diagnostics and/or treatment options.
If you are scheduling an appointment with our behaviorist, Dr. Michelle Posage, you may be asked to complete one or more forms prior to the initial visit. The following forms are available on our website:
- Canine History Form
- Feline History Form
- Feline Elimination History Form
Please contact Holly to determine if any of these forms are appropriate for your first visit.
New patients may be registered by telephone when your initial visit is scheduled. Your family veterinarian will be asked to forward your pet’s history which we will carefully discuss with you during your initial consultation. |
